Spring 2012 Registration fee is $1500 + tax ($1725.00 tax incl.)

I hereby accept to pay a minimum deposit of $1000 before April 20th and pay the remaining amount (totalling $1725.00) due by June 3, 2012. As team captain, I am aware that I will be held fully responsible for any unpaid balance and will be charged an additional $50 for every week my balance remains unpaid past the above deadline.
